Payment-card information is processed by third-party payment providers and is not stored directly by Lost and Crowned, Inc.
Confident Crown application information may include hair-loss information, hair-loss photos, prescription or other medical documentation, information about a diagnosis or condition when voluntarily provided or required to evaluate eligibility, measurements and hair-replacement preferences, insurance-related information, referral information, and documents needed to assess the request. Please provide only information requested through the designated secure application process.
We may also collect technical information automatically, including IP address, browser and device type, operating system, referring pages, pages viewed, cookie identifiers, and general site-usage information.
2. Why We Collect and Use Information
We use information to respond to inquiries; review program applications; determine eligibility and appropriate support; communicate with applicants; coordinate available hair-replacement, fitting, styling, resource-navigation, workshop, peer-support, volunteer, donation, and partnership activities; process donations and acknowledgments; improve website content and services; maintain security; keep required organizational records; and comply with legal obligations.
Hair-loss photos, medical or prescription documentation, hair-loss information, measurements, and insurance-related information are collected only when relevant to reviewing a Confident Crown request, confirming eligibility or documented need, identifying an appropriate hair-replacement option, coordinating approved support, exploring available funding or coverage resources, and maintaining necessary program records.
3. How We Share Information
We do not sell, rent, or trade personal information. We may share information with Wix and other vendors that host or support our website, forms, communications, data storage, donation processing, and security.
When needed to evaluate or fulfill an approved Confident Crown request, we may share the minimum necessary information with authorized service providers or partners, such as qualified wig or cranial prosthesis vendors, fitting or stylist partners, referral partners, funding or benefits-navigation partners, and other professionals assisting with the requested service. Information is shared only for the relevant purpose and subject to appropriate confidentiality expectations or agreements when applicable.
We may also disclose information when required by law, legal process, or governmental authority; to protect the rights, safety, or security of applicants, the public, Lost and Crowned, Inc., or others; or in connection with an organizational transition where lawful protections remain in place.
Lost and Crowned, Inc. does not claim that its website, forms, or programs are HIPAA-compliant or that the organization is a HIPAA-covered entity unless and until that status is formally verified. Applicants should not send medical information through the general Contact form or ordinary email.
4. Retention and Deletion
We retain information only as long as reasonably necessary for the purpose for which it was collected, to administer programs and services, document organizational decisions and transactions, address disputes, meet legal, tax, grant, insurance, audit, safeguarding, or recordkeeping requirements, and protect organizational and applicant interests. Backup copies may remain for a limited period until overwritten through normal system processes.
